data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<p>Palliative care is available when you first learn you have a life-limiting or incurable illness.\u00a0 Palliative care is specialised medical care that focuses on providing people with relief from pain and other symptoms of the illness.\u00a0 It can also help people cope with side effects from medical treatments.<\/p><p>You might be able to receive palliative care while you are receiving other treatment for your condition.\u00a0<\/p><p>End of life care is a form of palliative care you receive when you\u2019re close to the end of life &#8211; in the last months, weeks, or days.\u00a0<\/p><p>Different health and social care professionals are involved in palliative and end of life care. Understanding how these professionals can help you and knowing what is available in your area can help you access the best care for you or your loved one.\u00a0<\/p><p>Care at the end of life should be centred around the person. The quality of care a person receives at the end of their life \u2013 and how and where they receive it \u2013 can make a huge difference to the individual and their family, friends, loved ones, as well as the people that deliver the care.<\/p><p>Most people will have a range of needs and it might take different members of staff working together to manage them all. \u00a0This is why someone who is living with a terminal illness will have different NHS and social care professionals, volunteers, as well as friends and family members, all involved in their care.\u00a0<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/section>\n\t\t\t\t<section data-particle_enable=\"false\" data-particle-mobile-disabled=\"false\" class=\"elementor-section elementor-top-section elementor-element elementor-element-38b873e elementor-section-content-middle elementor-section-boxed elementor-section-height-default elementor-section-height-default\" data-id=\"38b873e\" data-element_type=\"section\" data-e-type=\"section\" data-settings=\"{&quot;background_background&quot;:&quot;classic&quot;}\">\n\t\t\t\t\t\t\t<div class=\"elementor-background-overlay\"><\/div>\n\t\t\t\t\t\t\t<div class=\"elementor-container elementor-column-gap-default\">\n\t\t\t\t\t<div class=\"elementor-column elementor-col-100 elementor-top-column elementor-element elementor-element-def4a3e\" data-id=\"def4a3e\" data-element_type=\"column\" data-e-type=\"column\">\n\t\t\t<div class=\"elementor-widget-wrap elementor-element-populated\">\n\t\t\t\t\t\t<div class=\"elementor-element elementor-element-cf13b05 elementor-widget elementor-widget-heading\" data-id=\"cf13b05\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t<h2 class=\"elementor-heading-title elementor-size-default\">Planning care for people who have a life-limiting condition or are at the end of their life <\/h2>\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t<div class=\"elementor-element elementor-element-c9a6adb elementor-widget elementor-widget-text-editor\" data-id=\"c9a6adb\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<p>In Leicester, Leicestershire and Rutland, the local NHS have worked with a lot of other organisations to put together a document called \u2018<strong><em>Our Palliative and End of Life Care Strategy.<\/em><\/strong>\u2019\u00a0 We have also listened to what people, their families and staff have told us about receiving and delivering care, which has helped us to put the document together.<\/p><p>The Strategy describes what we believe people of any age need in the final stages of their lives to maintain as much independence as possible. Here is a short summary:<\/p><p>The Strategy includes a statement describing what we want to achieve in the long run from the Strategy. We have called that statement <strong>a vision<\/strong>, which says:<\/p><p><strong><em>\u201cWe will ensure you have a personalised, comfortable and supported end of life with personalised support for your family and carers.\u201d<\/em><\/strong><\/p><p>The Strategy has\u00a0<b>10 priorities<\/b>\u00a0which are listed below. These are the main aims that we would focus on and achieve.\u00a0<\/p><p>We will:<\/p><p style=\"padding-left: 40px;\">1)\u00a0 Improve health equity in palliative and end of life care service planning, provision and outcomes.\u00a0 (Equity recognises that everyone is not the same and will need appropriate support to address their needs).<\/p><p style=\"padding-left: 40px;\">2)\u00a0 Map (or clearly understand) currently available palliative and end of life care services provided in different place to any identify gaps.<\/p><p style=\"padding-left: 40px;\">3)\u00a0 Improve the services provided where gaps are identified, including for:<\/p><ul><li style=\"list-style-type: none;\"><ul><li>Bereavement support (helping someone cope with a death)<\/li><li>Anticipatory prescribing \u2013 this is medication prescribed in anticipation of symptoms to give relief from pain.<\/li><li>Social care offer \u2013 the services provided by socials care e.g. practical help everyday life.<\/li><\/ul><\/li><\/ul><p style=\"padding-left: 40px;\">4)\u00a0 Improve systems for early identification of people at End of Life.\u00a0 (Understanding earlier who needs support). \u00a0<\/p><p style=\"padding-left: 40px;\">5)\u00a0 Improve advance care planning.\u00a0 This is the discussion between an individual and their health and care provider about their preferences and priorities for their future care.<\/p><p style=\"padding-left: 40px;\">6)\u00a0 Improve record sharing and inter-operability of data systems across settings.\u00a0 (Making sure that everyone involved in a person\u2019s care can access information about them and their needs, so patients don\u2019t have to keep repeating it).<\/p><p style=\"padding-left: 40px;\">7)\u00a0 Improve care transition between settings. Making sure if people move from one service to another, that this is easy for the patients and their loved ones.)<\/p><p style=\"padding-left: 40px;\">8)\u00a0 Improve information, communication and engagement:<\/p><ul><li style=\"list-style-type: none;\"><ul><li>support available;<\/li><li>support to self-manage conditions;<\/li><li>access and signposting to information and services;<\/li><li>service offer;<\/li><li>promoting discussion about death and dying;<\/li><li>culturally appropriate information (being respectful and mindful of ethnicity, race, language, background, religion, gender and gender identity).<\/li><\/ul><\/li><\/ul><p style=\"padding-left: 40px;\"><br \/>9)\u00a0 Provide a consistent and comprehensive training and resilience offer for staff carers and volunteers (including recruitment), and monitor take-up. (Resilience is their ability to cope).<\/p><p style=\"padding-left: 40px;\">10) Improve the support provided to families and carers.<\/p><p>The Strategy explains an assessment that was undertaken by the Leicester City Council, Leicestershire County Council and Rutland County Council. The assessment outlines what is needed at end of life. <strong>It is divided into 6 ambitions. <\/strong>Each ambition has several actions that, if undertaken, will help to achieve the ambition. The ambitions are:<\/p><ol><li>Each person is seen as an individual.<\/li><li>Each person gets fair access to care.<\/li><li>Maximising comfort and wellbeing.<\/li><li>Care is coordinated.<\/li><li>All staff are prepared to care.<\/li><li>Each community is prepared to help.<\/li><\/ol><p><br \/>The Strategy lists <strong>6\u00a0different workstreams<\/strong> (groups of people working together on certain tasks) that will be put in place to deliver the actions within the Strategy. They will report to a more senior group called the Palliative and End of Life Task Force. This group then reports into various other groups, so that their work is accountable overall to the Leicester, Leicestershire and Rutland NHS Integrated Care Board. The people on the Integrated Care Board are senior managers from all the main NHS and local authorities in Leicester, Leicestershire and Rutland. It also includes organisations who represents patients.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/div>\n\t\t\t\t\t<\/div>\n\t\t<\/section>\n\t\t\t\t<section
