Zaburzenia erekcji (impotencja) definiuje si\u0119 jako niemo\u017cno\u015b\u0107 uzyskania lub utrzymania erekcji wystarczaj\u0105cej do penetracji i satysfakcji obojga partner\u00f3w seksualnych.<\/p>\n\n\n\n

LLR ICB sfinansuje leczenie w nast\u0119puj\u0105cy spos\u00f3b<\/strong>

1. Leczenie inhibitorem fosfodiesterazy typu -5, generycznym sildenafilem, w minimalnej skutecznej dawce zaleca si\u0119 ka\u017cdemu m\u0119\u017cczy\u017anie cierpi\u0105cemu na zaburzenia erekcji, przy cz\u0119stotliwo\u015bci dawkowania przez maksymalnie cztery razy w miesi\u0105cu<\/strong>.

2. W przypadku wszystkich innych inhibitor\u00f3w fosfodiesterazy typu 5 (wardenafil, tadalafil i awanafil) zaleca si\u0119 wy\u0142\u0105cznie pacjentom spe\u0142niaj\u0105cym kryteria rz\u0105dowego programu Selected List Scheme (SLS) I<\/strong> w przypadkach, gdy generyczny syldenafil jest nieskuteczny, poda\u0107 cz\u0119stotliwo\u015b\u0107 dawkowania maksymalnie cztery razy w miesi\u0105cu<\/strong> stosowanie leku o najni\u017cszym koszcie nabycia.

3. Leczenie prostaglandyn\u0105 E1 do cia\u0142 jamistych i wlewkami docewkowymi zalecane jest wy\u0142\u0105cznie u pacjent\u00f3w spe\u0142niaj\u0105cych kryteria SLS I<\/strong> tylko wtedy, gdy doustne inhibitory fosfodiesterazy typu 5 s\u0105 przeciwwskazane lub nieskuteczne. Maksymalna cz\u0119stotliwo\u015b\u0107 dawkowania powinna wynosi\u0107 cztery razy w miesi\u0105cu<\/strong> stosowanie leku o najni\u017cszym koszcie nabycia.

4. Leczenie kremem Alprostadyl jest nie s\u0105 regularnie finansowane<\/strong> ze wzgl\u0119du na ograniczone dowody na skuteczno\u015b\u0107 kliniczn\u0105 i kosztow\u0105.

5. W przypadku m\u0119\u017cczyzn, kt\u00f3rzy nie toleruj\u0105 terapii, nie mog\u0105 stosowa\u0107 terapii zastrzykami dojamistymi (ICI) lub \u015brodk\u00f3w doustnych, mo\u017cna rozwa\u017cy\u0107 leczenie za pomoc\u0105 urz\u0105dze\u0144 do erekcji pr\u00f3\u017cniowej.

6. Leczenie implantami pr\u0105cia b\u0119dzie finansowane w przypadku pacjent\u00f3w po prostatektomii i powik\u0142anego ostrego priapizmu. Operacja wszczepienia pr\u0105cia w schy\u0142kowym stadium zaburze\u0144 erekcji jest finansowana przez NHSE.

Leczenie interwencjami psychoseksualnymi mo\u017ce by\u0107 stosowane w pracy z pacjentami trzeciego stopnia, dlatego te\u017c lekarze pierwszego kontaktu powinni kierowa\u0107 do lekarza pierwszego kontaktu i prowadzi\u0107 go w ramach podstawowej opieki zdrowotnej w ramach opieki klinicznej.
