VITALINK SUPPORT LTD

VitaLink Support Ltd is a nurse-led community wellbeing and functional support service operating across Leicester, Leicestershire and Rutland. We focus on early identification of wellbeing decline, structured routine support, appointment assistance, companionship and follow-up to help prevent escalation and maintain independence. Our approach combines clinical insight with practical, community-based support, working collaboratively with families, VCSE partners and primary care where appropriate. We aim to reduce avoidable crisis, promote stability, and support adults to remain engaged, connected and independent within their local communities.
