This page is for patients who are living with diabetes or those who are at risk of developing diabetes and would like to  prevent it. You will find details below of the services and sources of support that are in place to help you.

Across Leicester, Leicestershire and Rutland, the prevalence of diabetes is higher than that nationally. As well as diabetes, we also have a higher prevalence of Gestational Diabetes, this condition leads to higher risk of developing Diabetes.

National Diabetes Prevention Programme

To help manage the risk of Diabetes and promote a healthy lifestyle, individuals with or have a history of Diabetes can access the National Diabetes Prevention Programme. This is a 9-month programme focussing on healthy diet, being more active and maintaining a healthy weight. The programme can be accessed via a self-referral or via your GP.

In addition, there is a separate programme for those with or a history of gestational diabetes. You can refer yourself to the gestational diabetes programme or be referred by your GP.

Healthy Living (digital structured education for type 2 diabetes)

Healthy Living is a free, online NHS programme that supports people to live well with type 2 diabetes.

It includes information and advice about:

  • type 2 diabetes
  • eating well
  • becoming more active
  • how to look after your body and mind
  • other NHS services and support
  • living with diabetes, including driving, working and travelling

Type 2 diabetes Path to Remission

This is a service for patients with type 2 diabetes who are overweight or obese. It is designed to help patients reduce their weight and stabilise their Hba1c. This 12-month programme is designed to support people to improve their diabetes control, reduce diabetes-related medication and in some cases, put their Type 2 diabetes into remission.

Patients can choose between delivery in-person in local venues or digitally using an app.  Both formats are one-to-one and include 12 months of coaching support.

Weight management

Some patients who have type 2 diabetes may also benefit from weight management services and support. 

NHS Digital Weight Management Programme for people with diabetes

The NHS Digital Weight Management Programme supports adults living with obesity who also have a diagnosis of diabetes, hypertension or both, to manage their weight and improve their health.

It is a 12-week online behavioural and lifestyle programme that people can access via a smartphone or computer with internet access.
